What Is Postpartum Pubic Symphysis Pain?

The pubic symphysis joint is the most forward-facing part of a woman’s pelvis, or pelvic girdle. It consists of 2 pubic bones and a cartilage disc that sits in between the two pubic bones. Postpartum pubic symphysis pain, also known as pubic symphysis diastasis or pubic symphysis dysfunction (SPD), happens when there is a gap between the two pubic bones after delivery. Normally, there is no gap between the bones.

But during pregnancy, your body releases a hormone called relaxin that among other things, softens the ligaments surrounding the pubic symphysis. This is to prepare your pubic joint to have the flexibility needed for labor and delivery. The pubic joint separates to allow your baby to be born vaginally. For most women, the joint gap closes after the baby is born. But for others, the separation of the bones is larger and causes acute pain.

Risk Factors for Pubic Symphysis Pain

Pubic symphysis pain can happen to any woman after giving birth, but your risk increases if:

  • You had a forceps delivery.
  • The birth was rapid.
  • You did physically strenuous work during pregnancy.
  • Increased maternal age.
  • You have a history of difficult deliveries.

Is postpartum pelvic pain normal?

A certain amount of postpartum pelvic pain is normal. You may feel sore, or a little achy. But the pain from a pelvic symphysis separation is not normal. A pubic symphysis separation can make it hard to walk, let alone take care of your newborn baby. You may not even be able to roll over in bed at night without extreme pain.

Symptoms of Pubic Symphysis Pain

There are several pain symptoms specific to pubic symphysis separation, including:

  • Shooting pain in your lower pelvis area
  • Lower back pain that radiates into your groin area, thigh or leg, or abdomen
  • Pain with certain movements such as standing on one leg or climbing stairs
